The student is a Trainee of MCPT (Melbourne College of Professional Therapists). They are not a fully qualified massage therapist. I, the undersigned understand that they are learning and will have interruptions during massage sessions.

If you cancel within 24 hours a $15 cancellation fee applies. You must pay this fee before you make another appointment. Another appointment may not be made until this cancellation fee is paid.

The $15 appointment fee is to be paid before you commence your session with the Student Trainee.

The medical history sheet supplied to you must be filled out with your true & correct full details on your first appointment.

Relaxation & Well-Being Services:

A client (you) can have a session if you suffer from anxiety, stress, muscle tension/strain, to improve a feeling of well being, relaxation and to reduce stress or to manage general aches and pains.

Relaxation & Well-Being Services:

Sessions will consist of report-taking details, questioning, assessment and evaluation of your appropriateness for the session to be provided with the Trainee. When appropriate a session will be undertaken, to approximately 45 minutes. In some cases a session may be reduced to 30 minutes – should the client arrive late.

Remedial Therapy Massage:

For a client that suffers from neck, shoulder, hip, back, leg conditions/injuries or strains, also a medical health condition that is known to improve with massage, such as oedema, poor circulation, lethargy and similar. No work related injury or traffic accident injury will be treated by a student.

In this case the $15 fee is refundable:

In most cases with conditions such as the following, Epilepsy, Hepatitis B, Palpitations, Heart Conditions, High Blood Pressure, Heart Pacer, Hip Replacements or Pregnancy - the Trainee will not be able to attend to the treatment session. This is a contraindication to massage & they are not at the level to massage the client with such conditions. In this case the $15 fee is refundable.

 

If the student is comfortable and the health condition is not detrimental to (the client’s) health, the student can then apply a 4 minute recovery break for the client every 10 minutes treatment. The $15 fee still applies.

A Trainee can at anytime cease an appointment if they are uncomfortable with the client’s behaviour in anyway, and at any time a client may be asked to leave the clinic, or if a health issue was not disclosed on the history regarding the client; in this case the $15 fee is non-refundable.

 

I the undersigned understand that health conditions and a health screening over the phone is inadequate and my attendance at the clinic is more appropriate for the student to identify or decide whether I am or am not suitable for a Student Clinic Treatment Session. Or perhaps referred on to a Professional Massage Therapist.

The Trainee may be interrupted at anytime to answer the phones, attend to reception and/or discuss client issues with other clinic staff. A staff member may attend the room at any time for an assessment. The door must never be fully closed during a Student Clinic Treatment Session.

We do not accept coins and only $5, $10, $20 notes are accepted. A $100 or $50 notes will not be accepted. No cheques or credit cards are accepted. Please accommodate our request; the aim is not to make this task difficult for the Trainee.

Please understand that students are here to learn and at the same time offer you a suitable session with appropriate quality. Should you have any physical conditions then you should seek advice and an appointment with our Professional Therapists.

The charges for our Professional Massage services are 1 hour $50, or ½ hour $35. We also have Chinese Medicine, Acupuncture, and Remedial Massage Therapy. These can be paid by WorkCover and if appropriate the TAC. In most cases Health Funds rebate these services; please make sure that our therapist has an appropriate Provider Number.

CONDTION: A client is able to have 5 student massages, then must attend a Professional Massage Session at $50 for 1 hour, then allowed to return to 5 student massages (this is the 5 to 1 rule), the cycle then continues. This gives our Professional Therapists the opportunity of quality control and management of Student Clinic clients and to assess Student’s abilities.

We are sorry to advise that no children under the age of 18 can be left sitting alone in the reception area or sit in a Consultation/Treatment Room or to be left alone in the car park outside at anytime. Children aged less than 18 years of age may not be serviced by MCPT Students, only Healthy Life Clinic Professional Services are allowed.
